Job Description:
SMART CLINIC is seeking a Nurse Practitioner (NP) for a dynamic team located in Crescent Heights, Calgary, AB. . As a full-time Nurse Practitioner, you will be responsible for collaborating with a physician to provide care to patients. Full-time position with a competitive salary
Responsibilities:
Facilitating live and interactive clinical assessments, treatments, and medication management in-person and via telehealth technologies
Making appropriate assessments, treatment plans, and referrals for patient problems
Prescribing and monitoring opiate replacement (Suboxone) therapy
Treating and managing members that are interested in moderating or abstaining from alcohol with medications such as Naltrexone and Acamprosate
Treating and managing the member's other co-occurring illnesses including, but not limited to, anxiety, depression, insomnia, and hepatitis C.
Providing patient education and referrals specific to patients' assessed needs and abilities
Utilizing good communication skills, verbal and written
Exercising good physical diagnosis skills
Maintaining accurate records on patients treated.
Participating in case reviews, consultations, and utilization reviews
Qualifications:
Criminal record check
NP license with CARNA
Administration:
Maintain electronic medical records for all patients
Organize and schedule patient appointments
Prepare for each meeting and maintain current documentation.
Knowledge of MedAccess is an advantage. Experience with other electronic medical records is an asset.
Provide customer service, answer phones, greet patients and families
Adhere to SMART Clinic Policies and Procedures
Maintain confidentiality of all client profiles and company information
Complete other assigned tasks as requested.

Responsible for notifying immediate Clinic Manager of any Health & Safety risks or concerns
